OxyContin® (Extended-Release Oxycodone): Indications, Dosage, and Safe Use

OxyContin® is an extended-release formulation of oxycodone prescribed for the management of severe, persistent pain that requires continuous, around-the-clock opioid treatment when alternative options are inadequate. Because of its potency and long duration, it must be prescribed and monitored by a licensed healthcare professional.

For opioid-naïve adults, therapy typically begins at 10 mg every 12 hours. Available tablet strengths commonly include 10 mg, 15 mg, 20 mg, 30 mg, 40 mg, 60 mg, and 80 mg, taken every 12 hours. Dose titration is individualized and performed cautiously, based on pain control, prior opioid exposure, and tolerability. Higher strengths (60 mg and 80 mg) are generally reserved for opioid-tolerant patients only.

Dose reductions and slower titration are often necessary for older adults, and for patients with kidney or liver impairment or those taking interacting medications. OxyContin tablets must be swallowed whole—they should never be crushed, chewed, or split, as this can cause rapid release and dangerous overdose. Common side effects include constipation, nausea, dizziness, and drowsiness; serious risks include respiratory depression and dependence.
